This is a community based program for adults with physical or cognitive impairments and chronic health conditions. The program provides participants with a safe, pleasant, structured environment in which to engage in a variety of social, health and related support activities.
Each care plan is individualized to meet participant's special needs, in an effort to enable individuals to continue to enjoy an independent, healthy lifestyle.
